Change of Home InstitutionForm required to change your designated “home” institution, must be submitted by the 1st day of instruction.
Credit/No Credit OptionBroaden your education by taking courses outside of major requirements without affecting you grade point average.
Credit by ExamStudents who present evidence of having achieved course objectives through prior experience may apply for credit by exam or course challenge.
Diploma RequestIf you have already been awarded a degree/certificate and would like to order a degree/certificate, submit this form with a $25 processing fee to the Admissions and Records Office.
UH Dual Credit ApplicationRequired every semester for any Early College, Early Admit, or Running Start student.
Early Admissions ApplicationThis form, along with the UH system application are required for Early Admit, Home School students.
Enrollment VerificationTo verify your enrollment at Windward Community College.
Kingdom of Hawai‘i DeclarationUse this form to designate your affiliation with the “Kingdom of Hawai‘i” in the student information system.
Non-UH Transcript Forward RequestUse this form to request your non-UH transcripts to be forwarded to another UH System campus.
Request for Transcript EvaluationTo request an evaluation of prior coursework completed at another college or university.
Residency DeclarationTo apply for the status of Hawai‘i resident and receive the resident tuition rates.
Veteran Exemption to Nonresident Tuition• Veterans Post-9/11 GI Bill® (chapter 33) or Montgomery GI Bill® Active Duty (chapter 30) education benefits, who lives in Hawai‘i, and enrolls at the University within three years of discharge from a period of active duty service of 90 days or more, OR 

• Individual using transferred Post-9/11 GI Bill® (chapter 33) education benefits who lives in Hawai‘i and enrolls at the University within three years of the transferor’s discharge from a period of active duty service of 90 days or more, OR

• Individual using transferred Post-9/11 GI Bill® (chapter 33) education benefits, who lives in Hawai‘i, and whose transferor is a member of the uniformed services who is serving on active duty, OR

• Individual using Post-9/11 GI Bill® (chapter 33) education benefits under the Marine Gunnery Sergeant John David Fry Scholarship, who lives in Hawai‘i, and enrolls at the University within three years of the service member’s death in the line of duty following a period of active duty service of 90 days or more, OR

• Individual using educational assistance under the Vocational Rehabilitation and Employment (VR&E) (chapter 31) program and who lives in Hawai‘i.
